In this Article Review, Dr. Akihiro Ando discusses whether oral premedication can improve the success of inferior alveolar nerve block anesthesia in patients with irreversible pulpitis. The reviewed systematic review and network meta-analysis included 19 randomized controlled trials with more than 1,600 participants. The findings independently confirm previous evidence suggesting that oral premedication—especially NSAIDs—can increase the likelihood of successful IA nerve block anesthesia in irreversible pulpitis. The review also found that dexamethasone ranked highest among the studied medications, followed by NSAIDs and tramadol. However, the evidence supporting dexamethasone was more limited, and direct comparisons with NSAIDs did not show a statistically significant difference. Therefore, NSAIDs remain the best-supported evidence-based option at this time. Burning mouth syndrome may not represent a single pain disorder. This study examines whether patients with BMS show different underlying pain mechanisms, including impaired central pain inhibition in a clinically important subgroup. In this video, Dr. Akihiro Ando reviews a study published in the Journal of Oral Rehabilitation that investigated conditioned pain modulation, or CPM, in patients with burning mouth syndrome. The researchers compared patients with BMS and healthy controls and used responses to a lidocaine lingual nerve block to classify patients as having a presumed peripheral or central pain mechanism. The findings showed substantial variability among patients with BMS. Those with a presumed central mechanism demonstrated reduced pain inhibition at both the painful oral site and a control site, suggesting more widespread dysfunction of central pain-modulating pathways. Patients with a presumed peripheral mechanism showed a more localized abnormality. The study does not establish that impaired CPM causes BMS, and its small sample size and indirect classification method require cautious interpretation. Alzheimer’s disease has traditionally been viewed through the lens of amyloid, tau, and neurodegeneration. However, emerging evidence suggests that abnormal glycan metabolism may represent an important upstream mechanism contributing to disease progression. In this Article Review, Dr. Akihiro Ando discusses a new study investigating hyperglycosylation in Alzheimer’s disease using human post-mortem brain tissue, multiple mouse models, isotope-tracing experiments, and electronic health record data. The investigators found increased N-linked glycosylation in Alzheimer’s brains, demonstrated that enhanced glycan biosynthesis contributes to this abnormality, and showed that reducing glycosylation improved cognitive performance in mouse models. The study also raises important questions regarding glucosamine, a widely used dietary supplement. In animal models, glucosamine increased brain glycosylation and worsened memory-related behavior. Observational human data were directionally consistent, showing associations between glucosamine use and worse clinical outcomes in patients with cognitive impairment and Alzheimer’s disease-related dementias. These findings do not establish causation, but they highlight an area that warrants careful investigation.
